We have guardianship over our mom because she has dementia and our dad couldn't take care of her needs, but she has gained strength and can physically care for herself but not mentally. Our dads in physically incapable now, he keep falling and fractured him and shoulder, he's also been given a psychological evaluation and Dr's said he's not capable of decisions. My dad refuses rehab but he's in a rehab facility in Florida. Our mom can't be alone we fly down, taking turns from Northern Indiana/ Northern Michigan. Our parent's don't have much money for private pay assisted living, we need help to find medicaid assisted living and how does that work?

Find Care & Housing
You need to pick a state you want them to live in and visit a certified elder attorney to help you with the paper work. Each one must be physically and financially qualified to be placed in a SNF/NH. Some states allow Medicaid supplement in ALFs. Medicaid is state specific so you have three states to consider at this point.
Do you have POA for dad?
Do they have a home to sell?
Realize that after placement they will still need ongoing oversight.
This is a big job. Wishing you luck in getting them placed.

With dementia, your mom already needs your guardianship. Most people do not realize they need help. It is time to look at places and to determine about moving them before you cannot.

If you really don't have any idea I would now attend an elder law attorney with your questions. The POA pays for expert help. You need to gather all info about all assets. This is GUARDIANSHIP. VERY difficult to do long distance. I would hire a Licensed Fiduciary in your area. Elder law attorney will know good ones as they work with them through the courts when people do not have family. This is going to be tough and someone must be right there operating within the laws of that state. You need to be on all documents and accounts; that alone will be tough.

So off to the Elder Law Attorney. I was POA and Trustee for my brother by long distance. I would NEVER attempt that again. The first year was a nightmare of anxiety. Did I learn a lot? Yup. Would I ever do it again? Nope. And guardianship is WORSE because your legal responsibility is very difficult and you cannot be excused or allowed to quit except by the court and the court will never allow that.

I sure wish you luck but you will need legal help. That will spend down the last of their money nicely and hopefully you will find a good Medicaid facility that will take both of them together.

I wish you good luck. This will be hard. Get on your flying shoes because you need an attorney in their own state.
